This behavior by physicians with a choice of payment systems is not 'inadvertent', it is the expected behavior of physicians in a poorly structured capitated payment system. It boils down to risk and accountability; provide a way to minimize both while providing opportunities to maximize income and the results are inevitable, not inadvertent.
