Dean Baker's done stuff on this. Google promises a paper on the UNDP site, but it's not there anymore. His 1996 paper (a scan of a marked-up typescript - how low production standards used to be!) is at <http://www.lights.com/epi/virlib/Technical/1996/highc.PDF>.
