Mostly within Pakistan, if you ignore Kashmir, Afghanistan and Bangladesh (when Pakistan broke up).

May be not, but hundreds of thousands dead, if not more.

There is considerable violence in the 'periphery', but 'periphery' has yet to originate worldwars, holocaust, Gulags.
