Of course in general the agent can be the speaker himself: "I wish I could have studied harder for that test" (but I had to work late every day that week, the library was closed, the dog ate my lecture notes) vs. "I wish I would have studied harder for that test" (instead of drinking beer and playing Nintendo with my buddies).
