All new subscribers are automatically on moderation. I almost always clear the moderate flag after the first post, though in a couple of cases I've waited with some people I'm not sure about. Otherwise, no one else is on moderation. And I don't remember ever putting anyone on moderation before.
I'm guessing everyone can check his/her settings with the web interface, but I've only looked at the moderator's version of those screens, so I'm not sure.
