I've never heard this before.
Some of the pillow soft euphemisms I've come across in my illustrious career as a girl loving Blackamoor: "Passed away", "taken by God", "called home", "asleep" and "give me the keys you drunk ass bastard".
As Shane says, "transition" is most likely a WBAI thing.
