He's making a distinction between a real musician's life and the fantasy musician's life. It's not that famous musicians aren't real musicians. It's that they are unreal models for musicians. We know all know lots about their lives, but to aspire to it is to aspire to an unrealizable fantasy that even if attained, usually only last a very short while. Meanwhile most people know little about the how the 99.9% musicians not living like that live for their 40 year working life. Which is what the book is meant to explain.

Anyone who keeps on playing is a real musician in Chadbourne's book.
