I think it comes from writing too much in a hurry. My old work orders got less ligiable over time...as did my notes in fast pace classes. Docs also use a lot of abbreviations.
And, they might not be very literate. They go through a science like biology or chemistry which has fewer humanities and a minimum of English, and then medical school which is just a nightmare on speed.
