I've wondered if that had something to do with the difficulty in recognizing and treating it. From what I've read, there is a tendency for them to abandon treatment after the crisis which brought them there has passed, because that would require admitting something needing to be fixed.
Around the time I dug into the subject I found a couple of web forums for people with clinical narcissism - unlike the lively forums associated with other unflattering disorders, the few existing posts were along the lines of "LOL I got diagnosed with NPD".
