[WS:] You lost me. So they do not know what increases the probability of pilot error? They do not train them to avoid that?

Singular events have causes and those causes can be determined. You can determine them in probabilistic terms if the event occurs frequently enough or in descriptive terms, by enumerating a series of events that led to the mishap. It does not matter if that mishaps is an air traffic controller falling asleep or person snapping and going on a shooting spree - in each case you can, at least in principle, establish a series of events that led to it, no?
