Singular events have causes and those causes can be determined. You can determine them in probabilistic terms if the event occurs frequently enough or in descriptive terms, by enumerating a series of events that led to the mishap. It does not matter if that mishaps is an air traffic controller falling asleep or person snapping and going on a shooting spree - in each case you can, at least in principle, establish a series of events that led to it, no?
