Not in exchange for anything material, that's right. Because there isn't anything material the doctor can't get free of charge.

How is it done now do you imagine? Hold the threat of starvation over their heads and they immediately determine to forgo any income for several years in the hope of successfully negotiating the education minefield. Yeah, I see what you mean, that's a really great system for selecting and motivating those who you would entrust your life and health. Hard to imagine anything could be an improvement on that.

Same as now. We're only discussing a different means of motivating the producers here, not replacing them with aliens from another planet. At least that's what I thought was the question.
